Evaluative framework for phase II studies in patients with heart failure and preserved ejection fraction.

چکیده
Heart failure with preserved ejection fraction (HFpEF) is now recognized as a major public health problem, accounting for approximately 50% of all patients with HF (1), and morbidity and mortality rates in HFpEF approximate those of HF with reduced EF (HFrEF) (2). Aside from supervised exercise training programs (3), evidence-based treatments are unfortunately lacking in HFpEF. Challenges in the design and conduct of Phase II clinical trials may explain why

Effect of Exercise Training on Heart Rate Variability in Patients with Heart Failure After Percutaneous Coronary Intervention
Background: This study aims to evaluate the effect of exercise training on heart rate variability (HRV) and to determine the correlation between parameters of HRV and the ejection fraction in patients with heart failure after percutaneous coronary intervention.Material and Methods: Fifty patients with left ventricular ejection fraction ≤ 40% undergone percutaneous coronary intervention were r...
متن کاملFacts and numbers on epidemiology and pharmacological treatment of heart failure with preserved ejection fraction
Heart failure with preserved ejection fraction (HFpEF) is a major and growing public health problem. Epidemiologic studies demonstrated that heart failure (HF) can be clinically diagnosed in patients with normal or preserved left ventricular ejection fraction. These patients are therefore termed as having HFpEF. In the past, this was often called diastolic HF. Because of the permanent increase ...
متن کاملReductions in N-Terminal Pro-Brain Natriuretic Peptide Levels Are Associated With Lower Mortality and Heart Failure Hospitalization Rates in Patients With Heart Failure With Mid-Range and Preserved Ejection Fraction
Heart failure (HF) is estimated to affect ≈2% of the worldwide population, with an incidence approaching 10 per 1000 persons per year. These numbers are expected to increase with population growth projections, especially of the elderly. Half of patients have preserved (HFpEF) or mildly reduced (mid-range: HFmrEF) ejection fraction (EF). Overall prognosis is as poor as in reduced EF (HFrEF), whe...
متن کاملEffect of Spironolactone on Diastolic Function and Exercise Capacity in Patients WithHeartFailureWithPreservedEjectionFraction
HEART FAILURE (HF) WITH preserved ejection fraction (EF) accounts for more than 50% of the total HF population. Community-based cohort studies have shown that mortality rates are similar in HF with preserved EF compared with HF with reduced EF, but data from large clinical trials point toward a better outcome in HF with preserved EF. This may indicate that comorbidities that are typically exclu...
متن کاملHeart Failure With Improved Ejection Fraction: Clinical Characteristics, Correlates of Recovery, and Survival
Current guidelines for the management of heart failure (HF) divide patients with HF into 2 categories: those with reduced ejection fraction (EF of ≤40%; HFrEF) and those with preserved ejection fraction (>40%; HFpEF). It has been recognized that a subset of patients with HFpEF previously had HFrEF. These patients with recovery or improvement in EF (HFiEF) may be clinically distinct from those w...
